Health Requirements for Tolland Public Schools
Physical – A physical exam isrequired within 1 year of entering Tolland Public Schools. The Parent/Guardian is to complete the parentportion (front page) of the Health Assessment Record form and the Physician isto complete the physical and immunization information. The physical must include: height, weight, bloodpressure, postural and dentalassessments, hemoglobin/hematocrit, vision/hearingscreening, Chronic Disease Assessment, Immunizations, and date of the physical.
